	Manuscript form
	Candidate’s contribution to the project

PUBLICATION CRITERIA:

	Manuscript must be offered to the editor of Muscle & Nerve for first review.
	Manuscript must follow Muscle & Nerve author guidelines and be concise, with text no longer than 2500 words

DEADLINES & SUBMISSION GUIDELINES:

	Abstract deadline – March 1, 2009 (follow abstract guidelines) Abstracts must be submitted online
	Full manuscript must be submitted on CD-ROM to the AANEM Office by March 15, 2009
	Letter from program director certifying eligibility
	Letter from candidate describing his/her specific percentage of contribution to project
